Requirements and Directions for 2012
Amount: $2,000 one-time award

Eligibility Criteria: Graduating high school senior that has had an immediate family member stricken with lung cancer either in the past or present. The student needs to be graduating in the 2011-2012 school year and must be from Lake Travis High School, Dripping Springs High School, or West Lake High School. Applicants are required to have a minimum GPA of 2.5 on a 4.0 scale and must already be accepted by an accredited college or university. |
Interested students should complete the following:
- Print and fill out Bob Mayberry Scholarship Application Form Application Form
- a 500 word essay describing the impact of lung cancer on their life Essay Template
- an OFFICIAL TRANSCRIPT with grades and cumulative GPA posted through Fall of 2011
- two letters of recommendation, non-family members

Deadline: February 1, 2012
The winner will be notified by phone and email by February 15, 2012. The award will be presented in person at the Bob Mayberry Foundation Fundraiser, April 7, 2012 at La Paloma in Lakeway, Texas. The recipient will be asked to either provide a two to three minute video or give an acceptance speech regarding lung cancer and its effects on their life and how receiving the Bob Mayberry Foundation Scholarship will impact their future.
The video will need to be received by March 30, 2012.
